Tension Headache
Tight band, pressure behind eyes, throbbing

Prodromal visual symptoms are vague

Tongue tingling, vertigo, diplopia (double vision), transient visual disturbance, syncope, dysarthria (slurred speech), ataxia ( unsteady gait)

Mimics stroke, recovers within 24 hours

SAH, meningitis, thromboembolic TIA, sensory epilepsy

Cluster headache
Unilateral pain, clusters around one eye, vomiting, ipsilateral lacrimation, usually men aged 30-40

Paracetamol, metoclopramide (antiemetic), triptans (5HT receptor agonists)

When to consider prophylaxis
2+ attacks per month, increased headache frequency, significant disability, unable to take treatment medication

Migraine prophylaxis
Pizotifen (5HT antagonist, antihistamine), propranolol (B blocker), amitriptyline (tricyclics), sodium valproate, SSRIs, verapamil, topiramate, naproxen

Cluster headache management
Subcutaneous sumatriptan, verapamil (Ca channel blocker), topiramate, lithium carbonate (antiepileptics)
