What is the Mechanism of CxSp disc herniation?
Sustained repeated cervical loading, most commonly affecting cervical (C) 6–7 level. (Hammer et al., 2016 Study discussing epidemiology and pathophysiology)
1 of 8
How does CxSp Disc Herniation Present?
Pain- Specifically in cervical region (Depending on the nerve, pain may radiate only into the shoulder or may radiate down the arm into the fingers) Decreased ROM- Pain on extension as this decreases space in the foramen further compressing the nerve
2 of 8
Describe the Pathophysiology involved in CxSp Disc Herniation?
4 Types of Herniation- Disc Bulge- Disc Extends beyond end plate- Protrusion- A defect in the annulus- Extrusion- Disc Material extends beyond annulus- Sequested- A piece of the disc that breaks away from the main disc structure
3 of 8
Subjective assessment CxSpine Herniation- What information would you require about the pathology?
Symptoms- Location-Pain Scale- Pain type? Referred, Vague = Nerve- Referral Patterns-SIN FACTOR-Severity, Intensity and Nature of the symptoms. Sleep- Breathing Problems- Digestive Problems-Altered Sensation
4 of 8
CxSpine Herniation- What information do you require about the history of the present condition for the pathology?
History of injury- Mechanism- Heavy Lifting, Overuse- Age related? MAINLY A DEGENERATIVE CONDITION
5 of 8
CxSpine Herniation-What information do you require about past medical history for the pathology?
Previous head injury- 5D’s- DISPHAGIA, DISARTHIA, DIPLOPLIA, DIZZINESS, DROP ATTACKS- Fracture- Tumour- VBI- RA/OA- THREAD- MRI- X RAY-Cauda Equina Syndrome
6 of 8
CxSpine Herniation- MEDICATION
Medication- Pain killers before treatment? Mask Symptoms- Dosage? Frequency? Over counter , Prescribed?
7 of 8
What Special Test would you use for for CxSpine Herniation?
Spurlings Test- Compresses nerve root- Do not perform if the patient has radiating symptoms or Aggrevates.
