Advantages

Cheap
Easy
Quick
Can give you a wide range of information
Allows you to question people you may otherwise not be able to
Gives people the chance to remain anonymous

Disadvantages

Low response rates
Risk of untruthfulness
They can be rushed
Aren't always taken seriously
You don't get any help
People might not be bothered to return them

Evaluation

Overall questionnaires are a good way of gathering information if they are answered as you can make the questions very specific however as they are not done face to face you cannot trust the results.
