Haemostasis
arrest of bleeding . minutes. temp repair, coagulation cascade initiated, platelet aggregation adhesion, plug
1 of 10
Inflammatory response
eliminate microorgansism. removal of debris and dead cell, healing signals (growth factors, cytokine)
2 of 10
Diapedesis
inflam cells squeeze through blood vessel wall
3 of 10
Neutrophil
arrive in minutes. phagacytosis. generate ROS (respiratory burst) secrete lysosomal enzymes = bacterial destruction. Proteases = break down damaged tissue. peak in 3 days. extruded in scab/phagacytosed/ lymph system
4 of 10
monocyte and macrophage
secondary stage. diapedesis. activation [classically = IFN-1 (proinflam), alternatively = IL-13 (repair)]. activated monocytes become macrophage. peak day 7. release growth factors TNF- alpha, TGF-beta, FGF
5 of 10
Tissue regeneration
multiplication (fibro, keratino, endothe). Fibroblast = temp matrix (eventually replace by collagen and elastin) granulation 4 days. epidermis restored prolif and migra keratinocytes. fibro = wound contraction
6 of 10
Proliferation and angiogenesis
dense network of capillaries (1st few days). deliver inflam cells, O2, nutrients. intiated = invasion of endothelial cells. endothelial cells undergo apoptosis and blood vessel density decreases as new tissue formed
7 of 10
re-epithelialisation
prolif and migra kearatinocytes. 18 hours post wounding. they break cell to cell contact "leap frog" migrate over. cease once wound closed
8 of 10
Wound contraction
Dermal fibroblasts into wound bed. contractile phenotype = myofibroblast (7-14 days). produce contractile protein alph smooth muscle actin = alignes across wound bed and contract wound brought together
9 of 10
Tissue remodelling
collagen deposited by fibroblasts. matrix continuously turned over by collagenase. collagen fibres become cross linked and form layer bundles. resemble unwounded skin structure. mature colagenous scar 1-2 years. 80% original strength
