- High trauma
- Deranged vitals
- Head
- Within 1 hr
- GCS < 13 on initial assessment
- GCS < 15 2 hrs after event
- Suspected open/ depressed skull fracture
- Battle's sign, panda eyes, haemo-tympanum or CSF leak
- Post-event seizure
- Focal neurology
- 2+ episodes of vomiting
- Within 8 hrs
- Over 65
- Bleeding disorder
- Includes anti-coagulation and some antiplatelets
- 30+ minutes of retrograde amnesia
- Dangerous mechanism (RTC, fall >1m/>5 stairs
- Within 1 hr
- C spine (within 1 hr)
- GCS < 13 on initial assessment
- Intubation
- X rays inadequate
- X ray looks abnormal or suboptimal
- Definitive answer needed (e.g. before surgery)
- Multifocal CT required for assessment
- Alert and:
- Over 65
- Dangerous mechanism
- Focal neurology or paraesthesia in upper limbs
- Chest
