They damage the cells of the host, by rapturing them to release their nutrience, break down their nutrience inside the cell, or replicate inside the cells, bursting them

Harmful toxins.

How do pathogens enter through the gas exchange system?
most are trapped in the lining of mucus, then are removed by the cilia that beat the mucus up the trachea, and removed, but some pathogens are still able to reach the alveolar, where they can enter the cells and cause damage.

How do pathogens enter through the skin?
Damaging the skin means they can enter into the blood stream, platelets clot the blood to prevent this, however some might get in before the scab forms

How do pathogens enter the digestive system?
Eating or drinking them means they normally killed by the acidic conditions in the stomache, however some may survive and pass into the intestine, where they can invade the cells of the gut wall and cause disease.
