What is the main causative organism of primary TB?
Mycobacterium tuberculosis
1 of 11
what kind of response is there first of all at the site of infection and where does the infection spread after that?
There is an initial mild inflammatory response followed by a spread to regional lymph nodes
2 of 11
What is the 'primary complex' in TB?
It is a term which describes both the infective focus and lymph node involvement.
3 of 11
What is the infective focus known as in the lung?
the Ghon focus
4 of 11
Where is the Ghon focus (infective focus) located normally? ___-pleurally in the ___ zone of the lung. Can you name 1 other?
The common sites for initial infection are sub-pleurally in the midzone of the lung. In the pharynx or terminal ileum
5 of 11
How long after initial infection will the tissue reaction at the primary complex change to begin to form granulomas?
1-2 weeks
6 of 11
What happens in the majority of TB cases?
The caseating granulomas heal with fibrosis (they frequenlty calcify without any medical intervention)
7 of 11
What happens if viable bacteria bacteria remain walled off within the primary complex?
It becomes latent TB
8 of 11
There are many ways in which TB can become symptomatic. Erythema nodosum is common with primary TB - what is this?
A condition where there is inflammation of the fat cells under the skin resulting in tender red nodules on the skin (normally on both shins)
9 of 11
Name 2 other ways primary TB may become symptomatic
Ghon focus erodes through pleura and discharges organisms causing pleurisy. Enlarged hilar lymph nodes causes lung/bronchial obstruction/collapse or they erupt causing TB bronchopneumonia. Englarging nodes may erode to vessels = miliary dissemination
10 of 11
What is miliary tuberculosis? Where does it spread to if it gets into a) pulmonary arteries b) pulmonary vein
This is disseminated TB (spread) in the pulmonary artery it will disseminate to the lung (seen on CXR) and in pulmonary vein it will disseminate to systemic circulation
