Structured Questionnaire
A researcher uses a list of pre-determined questions and participants choose their response from a list of fixed alternative responses which can be collated and analysed.

Semi-Structured self report
A researcher uses a list of pre-set questions but the participants' responses are not fixed as they can express their own views.

such questions invite people to describe what they think or feel about a topic in their own words.
