They were afraid of recieving a fixed salary, this was solved by saying doctors would be paid for each patient treated and they could have private patients.

Why was public health finally improved?
The decisive change was the 1875 Public Health Act, this forced local authorities to provide clean water, proper drainage and sewers. Medical Officers of health must be assigned at this point too.
