Equilibruim is where both concentrations are the same, same amount of particles therefore they don't move anymore.

Osmosis is the movement of water from a high cocentration to a low concentration, down a concentration gradient through a semi-permeable membrane. It is passive.

Active transport is the movement of particles (minerals) from a low concentration to a high concentration, against a concentration gradient. It is not passive and it uses a protein pump.
