effective drug treatment of schiz (chemo). drugs effective in treating most disturbing forms of schiz are called antipsychotics. (allow patient to function more effectively, but dont 'cure' the patient)

CONCENTIONAL ANTIPSYCHOTIC DRUGS (CAPs)
treat the positive symptoms (hallucinations, thought dissorders). positve symptoms due to overactive dopamine system. (anti psychotics reduce dopamine levels). they are anatgonists (block not stimulate receptors).side effects (TD, 30%,75%perminant)

treat negative and positive symptoms, think works on dopamine and seratonin(not everyone agress). KAPUR AND REMINGTON act on D2 receptors, temprarily blocking them then breaking down to let normal dompanie levels=responsible for lower side effects.

JESTE ET AL found TD rates, after 9months, of 30% CAPs and 5%AAPs, this means that AAPs more appropriate than CAPs as less side effects=longer treatment

AAPs= DAVIS ET AL 3519 p's, relapse ocured 55% of p's with placebo, but 19% on AAPs.ROSS AND READ d et al missleading , 45% of placebo had no relapse. R & R argue perscriptions reinforce that there is 'something wrong', prevents stressor (find/deal)

introduction of AAPs raised expectations, meta-analysis of studies found that not much evidence supports superiority. LEUCHT ET AL only two new drugs were more effective than CAPs while two others were the same

most widely identified treatemtn of SZ. basic assumption id maladaptive thoughts distort beliefs and behaviour (eg). therapist lets p's develop coping stratagies/alternative explanations.CBT successful when therapist accepts p's 'reality'

not everyone benefits from CBT , K&K p's deemed unsuitable as would not fully emerge, older loss suitable than young. CHADWICK AND LOWE CBT reducted delu in 10/12 p's, helped 70%, other 30% deteriorated.

methodology limitations, brief treatment periods and little follow up,little emphasis of combo treatemtns. few studies on CBT alone, and need more to ain clearer picture.
