Taking a Medical History

• introduce yourself - identify patient and gain consent to speak with them
• take notes - ask patient's permission to do this
• presenting complaint - what the patient tells you is wrong i.e. chest pain
• history of presenting complaint - gain as much information as possible about the specific complaint
