A questionnaire is a prepared list of questions and answers used to find out a participants behaviour or opinions that can NOT be directly observed.

They are often standardised>E.G. in a fixed order OR in the same way for every participant.

They can contain open or closed questions.

Closed questions have a fixed no. of options as answers.
