The metaphor 'the fractured rudder of his shoulder-blade', also suggests that this person is lost or misdirected since a boat's rudder is used to guide and control its path. The word 'porcelain' brings to mind delicate, deathly-white china and for me at least makes me think of china dolls with their unnaturally and frankly disturbing complextions. The poet even includes some  militaristic imagery in this series of metaphors, such as 'parachute silk', 'unexploded mine' which, like the poem Nettles recalls memories and images of conflict.
