Inflammation of glomerulus from sepsis

Polycystic kidney disease (genetic) 

Heamodialysis 

Blood flows through narrow tubes separated from dialysing fluid by a partially permeable membrane

Small molecules diffuse from the blood to the fluid (and from fluid to blood)

Blood flows in opposite direction to fluid so a concentration gradient of urea is maintained
