Medial head of clavicle, manubrium, sternum body, costal cartilages to bicipital groove  and shaft on humerus

Sacrum, transverse processes of all vertebrae, occipital 

spinous processes, occipital, acromion and spine of scapula

Lateral part of clavicle, acromion and spine of scapula. Deltoid tuberosity of humerus 

Spinous processes of inferior thoracic, lumbar and sacral vertebrae, iliac crest, bicipital groove of humerus  

Coracoid process (scapula),

to radial tuberosity 

Anterior surface of the humerus to coronoid process (ulna) and ulna tuberosity

Medial epicondyle (humerus) and superior half of anterior ulna shaft 

Medial epicondyle (humerus), olecranon and posterior  border of ulna to pisiform hamate and thumb 

Medial epicondyle (humerus) to base of 2^nd metacarpal 

Infraglenoid tubercle of scapula, posterior surface of humerus (x2) to olecranon (ulna).

Lateral epicondyle (humerus) and posterior ulna border to dorsal base of 5^th metacarpal 

Lateral epicondyle (humerus) and extensor expansions of medial four digits. 

Lateral epicondyle (humerus) and supra-epicondyler space ridge (humerus) to 2^nd and 3^rd metacarpals

T12 and L1-5 vertebral bodies and iliac fossa to inguinal ligament to lesser trochanter (femur)

All lead to quadriceps tendon which attaches to tibial tuberosity (via patella) 

Anterior inferior iliac spine and acetabulum to quadriceps tendon at patella base

Lateral condyle and lateral surface of tibia to medial cuneiform and 1^st metatarsal

Posterior ilium and posterior sacrum and coccyx to lateral condyle of tibia and Gluteal tuberosity (above linea aspera)

Ischium tuberosity to linea aspera

Lateral and medial  condyles (femur) to posterior surface of calcaneus (calcaneal tendon)

Posterior head and surface of fibula, soleal line to posterior surface of calcaneus (calcaneal tendon)

Pubic bone and ischium tuberosity to linea aspera (femur)
