The first question is wrong. It is two nuclei and two or three neutrons that are released in a nuclear fission chain reaction.

for the first question, electrons don't come from the nucleus, they come from the outer shells of an atom.

the question is right - in beta radiation a neutron in the nucleus splits into a proton and electron. The electron is then fired out from the the nucleus as it repels the positive charge.

In terms of beta decay the electron comes from a neutron in the nucleus which then beconmes a proton.

Check question 2. The live and neutral should have an alternating current as they are part of the same circuit. Live will have an alternating p.d. while the neutral will have a p.d. close to zero.
