Pretty good quiz (good text diagrams).  A couple of issues though.

Question 19: Distance and MASS affects gravitational attraction, NOT size.  Consider a black hole.  Its gravitation attraction is colossal because it has a very large mass, but they can be the size of a matchbox.

Question 15: Not all objects are symmetrical.

Question 4: Virtual images form when IMAGINARY RAYS INTERSECT.  This can be behind a mirror, but it can also be formed by an object very close to a converging lens, and all diverging lenses form virtual images.

Question 11: The answer should be 'Sound waves with a frequency above 20000Hz'.  While your answer is typically true, older people generally can't hear frequencies above about 15-18000 Hz.  So what they can't hear may not necessarily be ultrasound.

Question 6: Refraction happens because LIGHT CHANGES SPEED when it crosses two DIFFERENT media of different densities.  Refraction will take place in the situation you said was the correct answer, but it is not the cause of refraction.

Question 1: Moment = Force x Perpendicular distance between the line of action of the force and the pivot.  Not just 'perpendicular distance'.

Other than that, just a few spelling errors, and a good quiz :)

the only thing i'd comment on is the fact that 3Nm is the same as 3N/m, so i wouldn't count them as two different answers
